We welcome you as a patient and appreciate the opportunity to provide you with quality medical treatment. The information that follows is designed to answer many questions most patients have. We want you to know about our policies and methods of practice. The more you know, the more we can be of service. If you have and questions, however, please ask.
From the south on I-65
Take Brook Street exit, drive straight past Chestnut Street at bottom of exit ramp, past traffic light at Muhammad Ali Boulevard; then turn right at next traffic light (Liberty Street). Entrance to parking lot will be on the right.
From Indiana I-65
Take Brook Street / Jefferson Street exit and follow Jefferson Street ramp. At bottom of ramp is Brook Street. Go past Brook Street and turn left at next light(First Street). Drive one block to Liberty Street. Turn left on Liberty Street, drive past Brook Street and turn right into parking lot.
Exit onto I-65 south, then follow directions from Indiana on I-65.
Everyone in this practice operates as a team member. All of us have been trained as professionals and we take pride in our professional capabilities.
Practice Hours: We offer appointments from 12:30pm - 4pm on Mondays and Wednesdays, 11:30am - 3pm on Fridays.
Appointments: We try to give each patient an appointment, and request that you call in advance so you can reserve time for you. The office telephone number is 502-584-6666 and all appointments are made at this number. We make every effort to honor all time commitments and request that you extend the same courtesy to us. On occasion emergencies can cause problems and, whenever possible, you will be informed of any delays. IF YOU CANNOT KEEP YOUR APPOINTMENT, PLEASE CALL THE OFFICE AS SOON AS POSSIBLE. You will be charged for breaking an appointment without notifying us.
Emergency Care: We recognize that you can have an emergency and we will do our best to respond to your problem promptly. In the event of an emergency, CALL THE OFFICE immediately. During office hours the staff will assist you. At all other times the answering service will direct your call to the physician on call. DO NOT CALL DR. KATZ AT HOME.
Telephone Calls: All patients are encouraged to call with any questions they have on medical problems but it would be unfair to our patients if the doctor left them to answer every telephone call. The office staff has been trained to answer many of your questions. They will also relay your information to Dr. Katz , who will return your call at the earliest opportunity.
After hours telephone calls: If you find it necessary to contact the doctor after normal office hours, the answering service will forward all messages to your doctor or physician on call. You call will be returned at the earliest opportunity. DO NOT CALL DR. KATZ AT HOME.
Prescription and Refills: All prescriptions and authorizations for refills must be requested between 9:30 AM and 4:00 PM Monday through Friday. NO EXCEPTIONS WILL BE MADE.
Fees and Payments: We make every effort to keep down the cost of your medical care. You can help by paying applicable fees, such as co-payments and deductibles, upon completion of each visit, and bringing current insurance information with you at the time of service. Payment arrangements can be made with our billing clerk, depending on special circumstances. Please let us know as soon as possible if you will need to set up a payment plan.
Insurance and Insurance Forms: We try to simplify the preparation of insurance claims, thereby holding down costs which are unrelated to the delivery of good medical care. We will file your insurance claims at no charge and process them quickly. In addition, our staff will file disability claims on your behalf with no charge for the first three and a charge of $25 per claim thereafter. If your insurance company requires its own form, you will need to supply it to us so we may file for you. PLEASE REMEMBER THAT IT IS YOU RESPONSIBILITY TO PAY ANY DEDUCTIBLE AMOUNT, CO-PAYMENT OR ANY OTHER BALANCE NOT PAID FOR BY YOUR INSURANCE COMPANY. We know questions can arise on insurance matters and these should be discussed with our insurance clerk.
Confidentiality: We believe that relationship with our patients is private and confidential and we will not discuss your case with anyone else without your permission. For patients under 18 years of age, findings and recommendations will be discussed with parents and/or legal guardians. Test results will be given only to the patients unless otherwise authorized.
A Final Word...please note that food and drink are not allowed in the office. We also request that you turn off your cell phone before seeing Dr. Katz.
